To improve patient access to care in a timely manner we have created this online request form. Below is the list of options for administrative requests that are offered by your practitioner without an appointment.
Prescription Refills - Current prescriptions only for stable conditions. Controlled substance prescriptions require an appointment.
Specialist Re-Referrals - For specialist follow up appointments. No new specialist referrals.
Letter Request - For simple requests. Example: letter for physio to submit to benefits. Private fees may apply.
Form Request - Depending on the form this may require a conversation with your doctor, but you may make your request here.
Please fill out the requested information under your family doctor’s name and allow 5-7 business days for your request to be completed. This service is not for new symptoms or a change in your condition. If your physician does not think it is safe to provide care for you in this way, we will call you for an appointment.
This form is NOT to be used for appointment requests. We strongly advise that you schedule appointments using the patient portal.
